Starting at Level 8, you are required to have a bathroom in your restaurant to keep your customers happy. If you have an open, clean bathroom that the customers can use, they will give you a thumbs up on the way out and your popularity will increase. If you don’t they will not provide you the thumbs up and you will lose out on possible popularity points. In fact, you can see which ones wanted to use the bathroom by seeing the toilet picture over their heads as they walk out.

Toilets get dirty every four uses, and require to be cleaned. The only way to clean the bathrooms is to have one of your employees either be a full time cleaner, or switch one from another duty to cleaning part-time. These cleaners will also clean up food left on the floor by customers and increase your coins as if you did it yourself, however when they are not in use they will not accomplish anything. If you watch them, they will appear to be walking around your restaurant cleaning but they will not actually be doing anything to further you in the game.

When it comes to making a bathroom for the toilet to go in, it is actually not necessary to have a wall up for the customers to use the toilet. Your restaurant city toilets can be used right in the middle of your restaurant, in plain sight of everyone. As far as I can tell, there is no penalty for this.
